Agreement of Membership and Informed Consent
On behalf of I confirm our membership of the British Arts Festivals Association (BAFA) for the calendar year of 2011. I also confirm our consent to BAFA publicising all information included in part 1 of this form in print and on the BAFA website, and to disseminating any information included in part 2 of this form within the BAFA membership for Festival, Corporate and Associate Members.
